Dr. Tenma faces a moral dilemma. He has to make a critical choice between saving the life of a young boy named Johan Liebert or the mayor of the city. Tenma decides to prioritize the life of the boy, only to witness a series of mysterious and horrifying events that follow. Johan, the boy he saved, grows up to become a sociopathic and charismatic young man.
